I got this guy and put him in my nano at work. He lasted just over a week before the mushroom in the last pic ate him. The mushroom got moved into the octo tank at the house.

I would guess that the water sitting in the bag for that long renders a test more or less useless. I'd go back to the LFS with a sample of your water for them to test, and describe the situation, and ask for a credit, or another animal, as long as your water tests fine. If they give you any...

Everything looks really good. The first one is probably alveopora. It looks nice and healthy. I can't believe that brain was ever unhappy, it looks fantastic. B
