How are your parameters currently? What corals are you housing? How many fish do you have? Generally, water changes of about 25%-50% in a nano is plenty and makes a protein skimmer unnecessary.

Go with the 150 watt Aquamedic pendant with a lamp around 14,000k. I think you will like it. It will be less intense towards the ends of the tank but not too bad. I had a 150 watt Hamilton Reefstar over my 20L and it worked well.

Of those three options, I would (and personally have gone with) go with option 1. I think that the perfect combo nowadays though is MH with T5HO supplementation.
Which corals are you looking to keep? Any plans for acro's or Croceas/maximas? If not, have you considered 2x65 watt PC...
I would personally go with the two of the model #4s. The flow is so dispursed on the koralias and i think your corals will appreciate the additional flow.

Yep. Those are the feeding tentacles. I fed them very small pieces of meaty frozen foods, shrimp, cyclopeeze, etc. Then just make sure none of the tissue is in contact with the LR where it can become damaged.

If I ever set up a large reef system I would definitely get an RK2 to automate everything. I have never heard of anyone regretting adding the RK2 to their system.

4x39 watts is more than enough for anything you would ever want to keep. I would suggest that. The only reason you would want to go with 6x39 watts is if you ONLY wanted to keep SPS and clams... but even 4x39 watts is sufficient for that... a 40g breeder is pretty shallow.
As far as lamps...
I love myself the starfire glass. While it's true that it is more noticable on the larger tanks, you can still see a bit of a difference on the smaller tanks. That being said, you have to figure out for yourself if that difference in clarity is worth the 3x or so more $$$ that you will be...
You should be able to use a razor blade or exacto knife and cut in between the polyps to frag those. You can cut the purplish mat and the colony will be fine.
