Fish dying in the same spot???

A friend of mine has a 5 or 6 gallon nano and 2 clown fish and another fish. The other fish died in this weird spot under a rock. Then one of his clowns died in the exact same spot. Aleast he found them that way. He wanted me to mention it. Said it was really weird.

Possible a hitchhike in the tank that is killing the fish and lives under that rock? A mantis shrimp perhaps.

that was my first thought. that they're dying and getting pushed into the same spot, but I didnt see either of them.
